As a Referral Coordinator, you will play a vital role in ensuring patients receive timely access to specialty care and healthcare services. This position serves as a key liaison between patients, providers, specialists, and healthcare facilities while coordinating referrals, authorizations, appointments, and follow-up care. Key Responsibilities
• Coordinate and process patient referrals for specialty services in a timely and accurate manner
• Verify and maintain accurate patient demographic, registration, and insurance information
• Serve as a liaison between Community Medical Group, specialists, healthcare facilities, and patients
• Perform eligibility and benefits verification as needed
• Obtain and process required referrals and authorizations, including Medical Director approvals when applicable
• Coordinate appointments, authorizations, procedures, and testing across multiple healthcare providers and locations
• Schedule appointments and arrange transportation services when necessary
• Communicate scheduling details and referral information to patients in a timely manner
• Follow up with patients to ensure completion of referrals and specialty appointments
• Coordinate pre-admission testing requirements with patients and clinical staff
• Enter referrals, authorizations, hospital services, and specialty care requests into the appropriate systems
• Monitor outstanding referral reports and follow up to obtain missing documentation and results
• Complete all referral-related administrative functions accurately and efficiently
• Maintain strict compliance with HIPAA regulations and patient confidentiality requirements
• Perform duties in an honest, ethical, and professional manner
• Perform additional duties as assigned
Qualifications
• High school diploma or equivalent required
• Previous experience in a clinical or healthcare setting preferred
• Experience with Microsoft Office applications required
• Medical Assistant training or certification preferred
• Knowledge of Electronic Health Records (EHR) preferred
• Knowledge of medical terminology, CPT, HCPCS, and ICD coding preferred
• Strong organizational skills and attention to detail
• Excellent customer service and interpersonal skills
• Strong verbal and written communication abilities
• Ability to work independently and manage multiple priorities effectively
• Strong problem-solving and critical-thinking skills
• Ability to maintain professionalism when interacting with patients, providers, and external healthcare partners
• Commitment to maintaining patient confidentiality and regulatory compliance
• Bilingual in English and Spanish or English and Creole preferred
